ROOT CANAL TREPANATION: A CLINICAL CHALLENGE IN ENDODONTIC TREATMENT.

Abstract

Objective: This study aims to explore one of the main endodontic accidents, root trepanations, as well as the causes and main therapeutic methods. Methodology: A literature review on root trepanation was carried out, one of the main challenges faced by Endodontists in their daily clinical practice. A search was carried out in the Pubmed database, using specific keywords, limiting the results to articles published between 2019 and 2024. Results: It was observed that the main triggering factors for this accident include complexity of dental anatomy, inadequate technical sequence and insufficient professional skill. Treatment should be conservative, depending on factors such as perforation diameter, contamination, hermetic closure and location, and is generally performed with repair cements that have biocompatibility with periodontal tissues, sealing capacity and alkalinity. Conclusion: According to the above, it is noted that root trepanations are caused by the use of endodontic instruments inside the root canal, such as drills, which in many cases can lead to tooth loss. Therefore, the most commonly used treatment is the Mineral Trioxide Aggregate (MTA) restorative cement. Periodic clinical and radiographic monitoring is necessary to verify therapeutic success.
