ORTHOPEDIC TREATMENT CLASS III WITH RAPID MAXILLARY EXPANSION AND USE OF A FACE MASK

Abstract

Introduction: Angle Class III malocclusion is considered one of the most complex orthodontic alterations, as it may involve skeletal, dental, and muscular impairments. Among its main characteristics is the anterior crossbite, frequently associated with maxillary retrusion. Hereditary, environmental, and craniofacial growth factors are related to the development of this condition, which can cause aesthetic, functional, psychological, and social impairments. Early diagnosis is fundamental to minimize aggravation and promote better therapeutic outcomes, especially during the active growth phase. Objective: To report early orthopedic treatment in Class III pattern patients due to maxillary deficiency, highlighting the benefits of interceptive orthodontics and the functional and aesthetic improvements obtained during growth. Methods: The methodology was based on a narrative literature review conducted across Google Scholar, SciELO, and PubMed databases, using articles published between 2005 and 2025, focusing on the orthopedic treatment of Class III malocclusion, rapid maxillary expansion, and the use of the face mask in growing patients. Results and Discussion: The Haas expander was cemented with glass ionomer; the activation protocol consisted of 2/4 turns morning and night for a period of 7 days, achieving palatal disjunction. Subsequently, a Petit face mask was installed for 18 hours of daily use with 300g elastics on each side. After two months of treatment, anterior crossbite correction and significant clinical improvement were observed. Early intervention demonstrated efficacy in stimulating maxillary growth, potentially reducing future surgical needs and favoring better functional and aesthetic development. Conclusion: Interceptive treatment with the Haas expander associated with the Petit face mask presented satisfactory results in correcting Class III malocclusion, highlighting the importance of early diagnosis, adequate orthodontic planning, and family cooperation for therapeutic success.
